If your account "Mon espace santé" is still active, you can also get this certificate.

If you do not have access to any of these services, you'll find information about TousAntiCovid app as well as a Frequently Asked Questions (FAQ) on this website government. It won't be possible to get the certificate from the Assurance Maladie in France.
