Document signed by CPAM

Bonjour everyone. I recently moved to France and just received a letter for the canton of Geneva asking me to fill up a document choosing Lamal or French insurance and then also having it signed by the CPAM in France, anyone could please tell me how to get this signed? I live in Gaillard, merci

Bonjour Criscort,

A person living in France and working in Switzerland can choose to be affiliated with:

  • the swiss system (LAMal);
  • or the french system (caisse primaire d'assurance maladie - CPAM).
You have 3 months after starting the activity in Switzerland or moving to France to make your decision. After those 3 months, you are automatically registered with LAMal.

If you decide to go with the CPAM, complete the form "Choix du système d'assurance maladie", specifically section 5: "affiliation au régime français d'assurance maladie" and attach the required documents.

Your CPAM will complete section 6 and give you back the form which you have to send to Switzerland within 3 months.

For the document to be completed by your CPAM, you can either:

  • send it to your local CPAM wich address is available on the ameli.fr website: "Adresses et contacts";
  • make an appointment at your local CPAM's office. To do so, I advise you to contact the CPAM’s English speaking line by dialing 09 74 75 36 46.
