birth certificate for carte vitale

Dear, I have sent three times the birth certificate of my husband in order to obtain its carte vitale because Asssurance Maladie (AM) did not received it. I am sure I sent it to the right address because I previously sent there all my documents. Last time I sent it, it was almost two months ago and I do not know if AM have received it or not. I would like to know if there is a phone number to which I can call or if there is some place in Bordeaux where I can go without RDV. For the moment my husband is not covered...Thanks in advance.
