First of all, the declaration of pregnancy must be drawn up by a midwife or general practitioner when you arrive in France. Actually, pregnancy declaration completed in the United States can not be accepted.

It should then be sent to your local caisse primaire d'assurance maladie (CPAM), together with your application for affiliation, depending on your situation.
