Where and how do I submit the E 104 form to benefit from social security from another European country in France?

I just moved back from Denmark after working there for 20 months to France. My danish health insurance gave me the E 104 form (in Danish) and told me that my periods of activity and social security contributions in Denmark and in France will be taken into consideration in order to open the right to social security allowances in France. I am 3 months pregnant and would need the French health care system to take my time in Denmark into consideration. Can you please tell me which documents I need to send to CPAM?

And do they accept the E 104 form in Danish? Or does it need to be translated?
