Sick leave document submission

Hai

I had an accident during my work and I was provided with sick leave for two months but I was not notified to submit the sick leave documents which is accident travail to the cpam , what should I do I heard that it is necessary to submit the documents to cpam within 48 hours. Can you please guide me in this situation

Hello akhil,

Doctors can send sick leave on line using your carte Vitale, with your agreement. He send only provide the third page that you must provide to your employer. 

If your doctor did not use this dematerialized service, he gave you three pages, you must send page 1 and 2 to your caisse primaire d'assurance maladie (CPAM) within 48 hours.

You can find your CPAM address on ameli.fr page "Adresses et contacts".

In case of an accident while working, your doctors provide a medical certificate explaining your injuries, called "Certificat médical initial", that is compulsory to send to your CPAM to get the instruction of your accident du travail started. On his side, your employer must send the "Déclaration d'accident du travail".
