If you are not able to finalize your application online as mentioned on this topic entitled: "How to be covered if I am not working and I am a European citizen ?", I suggest you send your application and supporting documents by post to your local caisse primaire d'assurance maladie (CPAM):

• A copy of your passport or identity card ;
  
• A copy of your birth certificate (the long version with the name of your parents) ;
• A proof of residency for the last three months (utility bills, rental bills ...) ;
• A copy of your income for the past year (private pension, savings, payslips...) ;
  
• A copy of your last tax form (even if not from France) ;
  
• A RIB (relevé d'identité bancaire, the bank details/IBAN) ;
  
• A U2 form if you receive an unemployment benefit paid by you previous state of employment ;
  
• The S1 refusal form from your previous health system ;
• The questionnaire form "recherche de droits" ressortissants européens inactifs (710) ;
• The form S1106 "Application for membership in the French health care system".
