How to close my CPAM account abroad?

Hello, please help me immediately.

I have to leave France for good on November 2022 because of family reasons and I don't know that I need to close my CPAM account. I was a student in France but didn't finish my study there. So, how to close my CPAM account, is it possible if I do it by PHONE? Or I need to send a documents (the Déclaration de transfert de résidence hors de France)? IF I have to send the documents, can my friends who still live there the one who send it? Do I need to send my CARD (the carte vitale) back? Thank you

In order to inform your caisse primaire d'assurance maladie (CPAM) that you have left France permanently, you must complete the form S1105 "déclaration de transfert de résidence hors de France" and return it by post to the CPAM in the departement where you lived when you were in France.

Your carte Vitale can be enclosed with this form.

I confirm this declaration cannot be made by telephone or by someone else as it is a declaration of honour.

You can find your CPAM's address on our website ameli.fr, section « Adresses et contacts ».

Thank you for your response.

To make sure, I have to send it by myself from where I am staying now (which is Indonesia), and I need to send the S1105 form (Déclaration de transfert de résidence hors de France) and my Carte Vitale by post to the CPAM where I used to live, which is in Angers,49100.
DO I also need to send like a cover letter to describe my situation on why I did not update about my situation earlier? Or are there any documents that I need to provide besides the S1105 form and the carte vitale?

Yes, that's exactly right.

If you want to enclose a covering letter explaining your situation, you can do so but it is not mandatory.

I confirm that there is no other document that needs to be provided in addition to your declaration.

Thank you for your response.

Just a quick question, after I send the documents and the CPAM of my department accepts it, will I be notified and get the "accusé de réception or some kind of emails to make sure that they received it? I just need to reassure myself because that's the only way they can contact me as I learned that I can't access my Ameli account and it always says " Une mise à jour de votre dossier est nécessaire pour accéder de votre compte. Merci de prendre contact avec votre caisse".

Your CPAM will not inform you when your application has been received or when it is being processed.

You will only receive notification by mail once it has been processed.

If you wish to track the progress of your application, you should contact them by phone.
