Issue with name clarification and required documents

I have sent my information in twice - even sending the same thing! I have finally received my social security number but it is requiring me to send in the same information (again) under a different name. I am married but I did not take my husband's name. I kept my maiden name. You are requiring me to send in information with my husband's last name but I don't have it because I never took the last name. Therefore, I cannot set up an Ameli account I am not Connie P. And I should not have to send in a dossier with that last name. I am my maiden name. How can you fix this? This will be the third time I have had to correct a situation where the documents sent in were not considered at all. Thank you.
