Change of address

You will receive a notification that your request has been processed. You will then have to wait for your new address to appear on your ameli account.

If you are unable to do this online, please send a copy of your identification (ID, passport) and a letter explaining the reason for your request, including your name, social security number and new address.

If you are leaving France permanently, you must inform your CPAM by completing form S1105.

If you are a Talent Passport, assistant de langue, mannequin or au pair, you will be register with the CPAM of Paris (75). Your application must then be submitted online or by post to one of the following addresses, depending on your situation :
