Create Ameli Account and Request Carte Vitale

I have received my social security number (starting with 2). I want to create an Ameli account and request my new Carte Vitale. It doesn't seem possible to perform these tasks online. I receive an error message when I try to create a new Ameli Account. Does this mean that the only way to request a Carte Vitale is to fill in the 'Demande d'ouverture des droits à l'assurance maladie' per post or by visiting a CPAM office? How to I get an appointment at CPAM? I have been trying to phone but no one answers. Is there a way of making an appointment online? Thanks.
