My COVID certificate has the wrong name?

I have my booster (third vaccine injection) 2 weeks ago and I found the COVID certificate has the wrong name. I immediately went to the vaccinator but he said that the record of the system was the name (which is wrong) and he cannot change it. So it means my COVID certificate of second vaccination is correct BUT the COVID certificate of third vaccination is NOT correct. I went to the doctor nearby but he still cannot change my name and he asked me to contact ameli.fr instead. I need to go back to my home country Hong Kong but the airline said my COVID certificate has wrong name so I cannot pass through the airline check in counter. It is very very urgent ! Please help me in this case otherwise I cannot go back to my home country. Please help ! Please help !.

Hello Pinky,


If you saw the error on the day of your vaccination, it is recommended that you go back to the vaccinator on the same day so that he/she can rectify the entry.

If you saw the error in the days following your vaccination, it is possible to ask a health professional (doctor, pharmacist, nurse, midwife) if he or she can modify the certificate.

As a last resort, you can also contact your health insurance fund, which may ask you for supporting documents if necessary.

I actually went back to the vaccinator on the same day but they said the system is showing the wrong name but they cannot correct the name. Then I went to the health professional who also said cannot correct my name. Both of them asked me to contact the Social Security Department in order to correct my name. Finally I try to create a ameli account but I failed. It is because to create ameli account need a 13 digits social security no. but my no. is 15 digits so I cannot create ameli account for further process. Can you reply to my email to teach me how to do ? Can you refer me to the suitable department to solve my name problem ? Now I am very frustrate because nobody can help me and I am stop in France & cannot go back to Hong Kong. I cannot book air ticket because my COVID certificate has wrong name !! it is very very urgent. Please help me. Please help me !! Please

Dear Sir,

I have my third injection of vaccines today 21 FEB 2022 and receive my updated COVID Certificate. However, I find that my full name is INCOMPLETE as show on BOTH the "COVID Certificate of third injection of vaccines" and "Summary of vaccination data against COVID-19". My full name on the COVID certificate is NOT correct. It looks like the Online System cannot show my full name correctly. I immediately went back to the vaccinator but he said the system only show the wrong name and they cannot help me. Then I also went to the health professional but he also cannot correct my name. So I have no solution. Please help me to AMEND my full name urgently. It is because I want to go back to my home country Hong Kong but I my COVID certificate has wrong name so I cannot book the air ticket. Please help me urgently. Please help me please !!

Please find attached the 2 documents for your reference.

For your information,I hereby also attached my COVID certificate of second injection of vaccines that I received on 6 JAN 2022 and also my Social Security temporary certificate which show my full name for your reference.

Please check & AMEND my name for my "COVID Certificate of third injection" and also the "Summary of vaccination data against COVID-19" accordingly

You do not mention which error has been made in your name.
I have the problem that my middelname does not appear on the certificate.Also impossible to change in France.Have tried everything same as you.
Hong kong does accept your certificate when the family name is correct and when one of your given names are mentioned. I am planning to go to HK this summer and am praying that things will go smoothly.
