How to get your first Carte Vitale

My wife applied for her Carte Vitale in March 2019. The problem is that the middle names on her passport are not shown on her birth certificate. She is UK citzen and would use the S1 form. She sent in a revised S1 form, received from NHSBS in the UK, that includes the middle names on her passport. But the local office at Cahors are asking for a UK notaire to show formal change of name. Now that the S1 form has all her names I do not understand why the Cahor CPAM should be delaying the issue of her Carte Vitale. She has a Carte de Sejour using the same names as presented to the Cahor office, so is it not possible to get a Carte Vitale using all her middle names. What would be the best way to get her Carte Vitale?
