Send request for Carte Vitale again and impossible to create Amelia account

Hello,

I have sent my letter requesting my carte Vitale via La Poste, but I'm afraid the address might be wrong. Is it possible for me to receive the letter with the form to send again? (the piece of paper with the place to sign and to put my photo).

I would rather do it online but when I try to create a compte ameli the request fails and it only tells me to be in contact with you. Why can't I create my ameli account even though I already have a social security number?
