Registration of child under parent card Vitale

If your child was born in India, you need to fill the S3705 form available on ameli.fr and send it to your caisse primaire d'assurance maladie (CPAM), along with a copy of your child’s passport and birth certificate.

You will find the address of your CPAM on the ameli.fr website : "Adresses et contacts".

When your child will be registered on your files, you will need to update your carte Vitale in a pharmacy.
