  <title>Type of Document Clarification</title>

&lt;p&gt;I hope this message finds you well. I recently checked my portal and saw that some of my documents were rejected. I apologize for only noticing this notification now. I am currently in the process of requesting new copies of the required documents on my end. I would like to kindly ask for clarification on the following points:	&lt;br&gt;&lt;/p&gt;

&lt;p&gt;1.) Aside from the “filiation dating from less than one year,” does my birth certificate also need to be translated into French?	 &lt;/p&gt;
&lt;p&gt;2.) Regarding the &quot;certification of registration for the current academic year&quot;, would the attached document be sufficient? It is called the Enrollment Assessment Form (EAF), which is the official enrollment document issued by my home university in the Philippines.&lt;br&gt;&lt;/p&gt;

&lt;p&gt;I would greatly appreciate your guidance on these requirements. &lt;/p&gt;

  <title>Reimbursment for feuille de soins without Ameli account</title>

So far, I had two appointments with two generalist doctors. The first appointment was not my medecin traitant and gave me a prescription and the second one signed me the &amp;quot;Déclaration de choix du médecin traitant&amp;quot;. Now I understand that I have to send these 3 documents (2 feuille de soins and Déclaration de choix du médecin traitant) to my CPAM but I still have some questions:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;Can I send all the documents in one envelope or should I send them in separate envelopes (per appointment)?&lt;/li&gt;&lt;li&gt;I have a mutuelle, and when I went to the pharmacy to take the prescription from the first appointment I didn&amp;#39;t have to pay anything. Should I still send the prescription to Ameli or is it not necessary?&lt;/li&gt;&lt;li&gt;Should I include any other document in the envelope (Attestation, ID card, etc)?&lt;/li&gt;Thanks!&lt;/p&gt;&lt;/div&gt;
