As long as the transport is more 150 km, a "demande d'accord préalable " must be made, unless if the treatment is linked to accident working or illness related to work.

If linked with long term illnes (ALD), a demand must be made prior to the transport.

The conditions I have been send state :-

Long-distance transport (more than 150 km one way), unless the treatment is related to an accident at work or an illness related to work, or a long-term illness (ALD);

Hello The Grocer,

If you disagree with the refusal for your transport, you have the possibility to contest as explained on the letter you have received.

However, if the ride between your place and the hospital is more than 150 km, your doctor should have had asked the agreement to your caisse primaire d'assurance maladie (CPAM) sending a "demande d'accord préalable" prior to the transportation.

Even if it is ALD if there is 150km or more you must apply for prior approval from your CPAM via "demande d'accord préalable" before transport:

The fact that it was issued under ALD adds to the question why I received the e-mail . As you say ALD is an exemption

Hello The Grocer,

Transport fees can be reimbursed by your caisse primaire d'assurance maladie (CPAM) upon medical prescription as explained in my previous answer.

However, in the following circumstances, you must apply for prior approval from your CPAM via "demande d'accord préalable" before transport:

- Long-distance transport (more than 150 km one way), unless the treatment is related to an accident at work or an illness related to work, or a long-term disease (ALD);
- Repeated transport (at least 4 transports of more than 50 km one way, over a period of two months, for the same treatment) unless the treatment is related to an accident at work, an illness related to work, or an ALD;
- Transport by plane or regular boat;
- Transport related to the care or treatment of children and teenagers in "centres d'action médico-sociale précoce" CAMSP) and (CMPP).

You can contact your CPAM to know more about your transport as you mention that it is under review.

Because if there is more than 150km you must ask BEFORE travelling if your CPAM is agree to pay the travel.
If you didn't ask before, they can refuse.
