If your prescription is dated on the day of your transport and you meet the conditions to be reimbursed, you can submit your claim by post if you are unable to do so online on your ameli account.

To obtain a refund for your transport costs, you must send to your caisse primaire d'assurance maladie (CPAM):

- the medical prescription for transport;

- the invoice issued by the transport provider, if it is a registered taxi, VSL or ambulance;

- form S3140 "Demande de remboursement des frais de transports pour motif médical en véhicule personnel et/ou en transports en commun", which can be downloaded from our website www.ameli.fr, for transport by private vehicle (enclosing proof of payment if it was public transportation).

You can find your CPAM's ccordonates on ameli.fr in this section: "Adresses et contacts".

The prescription issued by my doctor was issued dated the same date as the appointment, it is that the site would not allow me to complete the application after the travel.

Hello The Grocer,

The first condition for transport to be covered by the caisse primaire d'assurance maladie (CPAM) is that it must be medically prescribed and that the prescription must be issued before the transport.

If the prescription your doctor gave you is dated after the date of transport, your CPAM will not be able to process your request for reimbursement.

I advise you to contact your doctor again so that they can issue you with the document with the correct date.

You can then submit your reimbursement request via your ameli account or by post.
