Before you start, make sure you use the French version of the website (not the translated page).

To request your name to be changed from the app, go to “Mes démarches” > “Nom d’usage”.

If you log in from the web version of your ameli account, go to the section “Démarches” > “Modifier vos informations personnelles” > “Changer de nom d’usage”. Then, follow the instructions.

If you request is related with a marriage, you will need to attach supporting documentation (marriage certificate or family record book).

When you will see on your ameli account that your name has been updated, you can request for a new card.

For that, please send your previous card Vitale to your caisse primaire d'assurance maladie (CPAM), along with a letter explaining that your name has changed, they will send you a form when a new card Vitale can be ordered.
