You will receive a notification that your request has been processed. You will then have to wait for your new address to appear on your ameli account.

If you are unable to do this online, please send a copy of your identification (ID or passport) and a letter explaining the reason for your request, including your name, social security number and new address.

Moreover, if your bank account details have changed as well, you will have to send them at the same time with a copy of your old one.

All these informations have to be sent to the local Assurance Maladie office (CPAM) your are currently attached to. Can you find its adress on the page "Adresses et contacts" of our ameli.fr website.

If you are a Passport Talent holder, an "assistant de langue", employed as "au pair", or a model, wherever you are located in France, you must send your informations to CPAM of Paris to this address :

Notice that if you permanently leave France, you must inform your CPAM by completing the form S1105.
