Hi Annemarie,

To get the answer to your questions, I invite you to refer to the form notice "Application for membership in the French health care system" :

- A long-form copy of your birth certificate, or a short-form copy of your birth certificate showing your parents’ information, or an equivalent document issued by a consulate. (This document must be authenticated, specifically by a legible stamp.).

Civil status documents written in one of the following languages do not need to be translated: Albanian, German, English, Croatian, Danish, Spanish, Finnish, Hungarian, Italian, Latvian, Lithuanian, Dutch, Norwegian, Polish, Portuguese, Romanian, Slovak, Swedish, Czech, and Turkish.

If you send the original Birth certificate, your CPAM's office will keep it. You have to contact the CPAM to know if it will be possible to get it back.

Dear Lorraine,
Thank you for your quick response. I went to the link you sent me however, it was unable to answer the questions I had. Would you be so kind to let me know

if the birth certificate needs a certain issue date (i.e within the last 6 months, 1 year etc)?
whether I will receive the birth certificates back after sending them in since it takes quite a bit of money and time to get them?

For european citizens, there's no need to provide any official translation, except for Greek citizenship.
