your name;
your social security number (the first 13 digits);
your date of birth;
your postal code;
the last few digits of your bank account (the one you gave when you applied);
the serial number of your "carte Vitale".

Hello Hannah,

If you have a permanent social security number (starting with 1 or 2), you should be able to immediately create your ameli account with your bank account details (RIB - relevé d'identité bancaire) and your "carte Vitale". You need to fill in:

your name;
your social security number (the first 13 digits);
your date of birth;
your postal code;
the last few digits of your bank account (the one you gave when you applied);
the serial number of your "carte Vitale".

I advise you to contact the CPAM’s English speaking line. To do so, dial 09 74 75 36 46 (from France) or 0033 974 75 36 46 (from abroad).

For your information, the international call prefix or dial out code shown as "+" has to be replaced by figures which are different from the country where you’re located. You can find the list of these codes on the following link: http://www.indicatifs.fr/code-acces-international

You can also contact the French speaking line by dialing 36 46.

After validating your information, an adviser will send you a temporary code by text message which will allow you to login.

Hey Hannah,
I understand where you are coming from, but I'm not sure the social security employees can or are supposed to reply to questions asked in languages other than French...
I think you should try to do your best and reformulate it in French, even broken.
