The problem is that the middle names on her passport are not shown on her birth certificate.  She is UK citzen and would use the S1 form.  She sent in a revised S1 form, received from NHSBS in the UK, that includes the middle names on her passport.  But the local office at Cahors are asking for a UK notaire to show formal change of name. Now that the S1 form has all her names I do not understand why the Cahor CPAM should be delaying the issue of her Carte Vitale.  She has a Carte de Sejour using the same names as presented to the Cahor office,  so is it not possible to get a Carte Vitale using all her middle names.  What would be the best way to get her Carte Vitale?&lt;/p&gt;&lt;/div&gt;
