Hello Robin, I am having the same problem with E104. Were you able to get it in the end? If yes from where?

Hello,
thank you very much for your answer. At the very beginning I tried to call this number, but after severel days and more than 100 calls I still couldn't reach anybody, so I went directly to the office. As you can see from my very first question they sent me to Paris and the office in Paris back to Essonne.

You can go directly to the office. You will find the list of the offices close to you on the website ameli.fr : "Adresses et contacts".

Anyway, if you need some help regarding this request, you can contact the CPAM’s English speaking line.

For your information : the international call prefix or dial out code shown as “+” has to be replaced by figures which are different from the country where you’re located.

Dear Robin,

In order to get the E104 form (S1), from your local CPAM, I advice you to check with the center which covers you. Your center will appear on your attestation de droits.

You can print your attestation de droits and contact your center, by email, directly on your ameli account "Compte ameli".
