It is difficult to answer your question without being able to access your file.

Usually, a birth certificate issued in a non-European language must be translated. However, your local CPAM may ask you to provide a translation depending on the situation.

Regarding the school certification, only documents entitled ‘certificat scolaire’ are valid.

Reimbursment for feuille de soins without Ameli account

Hello Radu,

Yes, you can send all documents to your caisse primaire d'assurance maladie (CPAM) in the same envelope.

As you have not paid anything for the pharmacy prescription, you do not have to do anything with your CPAM about it, CPAM and your mutuelle have refunded the pharmacy directly.

You do not have to include anything health than the "Feuilles de soins" for the 2 consultations and the form to add your "Medecin traitant" on your file. Be sure that you have signed the "Feuilles de soins" bottom and that your social security number is written on top on "Numéro d'immatriculation".

You can have access to your ameli account and order your carte Vitale if your social security number has been certified.

Statements and receipts available on ameli.fr

You will find them under the section "Mes paiements".

Daily allowances payment statements are available for the last 8 years, and 6 months for disability pensions.

For any other statement, please contact your CPAM via "Espace d'échanges" section of your ameli account.

If you do not have an ameli account, your statements will be sent to you 4 months after your care has been paid for. Please note that your statements are automatically sent to the complementary health insurance (mutuelle) registered in your file.

Keep your allowances statements with no time limit, they will be necessary if you are entitled to a French retirement pension.
