My 87 year old Mother in law summited her application for a carte vitale with her S1 from the UK.

At the RDV we were told the dossier was complete/perfect but now 4 letters later and we are no closer to getting the carte vitale from Perpignan CPAM.

We have phoned twice and got no where. I have sent 3 recommande letters. The first 1A *** *** **** 0 with her childrens birth certificates which was not required at the RDV or in any of your applications.

The 2nd 1A *** *** **** 1 with her birth certificate and official translated version and the childrens birth certicates and marriage certificats. Her birth certificate and the officially translated one and her marriage certificate and an officially translated one were in the original dossier.

The 3rd demand 1A *** *** **** 9 I sent her brith certificates and translated ones AGAIN.

This is a joke, please give some staff training to the person working on this dossier. I will again send for a 4th time her birth certificates again today but I would like this carte vitale application to be processed as my mother in law is 87 years old and this is a simple transfer of her healthcare from the UK to France via the S1.

Oh and can you also get her NAME correct as per the application completed in your Perpignan office. The incompetence of the person actioning this file is beyond belief.

My husband phoned yesterday - we complained on the phone but we get nowhere with the action of my mother in laws dossier and we were told that we had never sent the birth certificate which is not true. You keep making spelling mistakes in my mother in laws name - despite us correcting it each time. We don't have a social security number for her as she has moved to live in France and you didn't give her a temporary number so we can't register her online to make a complaint. She is too old to take to the office to complain. Each time I send the documents back I write a message but I get no response - I ask them to phone or email me but this has not happened. You have my registration email from having to register for this forum. Please email me or google my name for my phone number.
